What to Bring to Your Appointment

To help us give you the best possible care, please remember to bring the following to your appointment:

  • Your valid health card and insurance information

  • A list of current medications

  • Your current glasses and/or contact lenses

  • A list of questions or concerns you may have

If you’re visiting us for the first time, arriving a few minutes early will give you time to settle in and complete any additional paperwork.
